The Medical Benefits Group of OOIDA offers a variety of benefits for you and your family. Each of these benefits have been carefully selected and tailored to meet your needs. OOIDA Voluntary Group Short Term Disability Plan OOIDA's Group Short Term Disability Plan protects you and your family against your short-term loss of income due to sickness or injury. There are two options available. Option 1 has a 30-day waiting period before benefits are payable. Option 2 has a 60-day waiting period. Both options pay a maximum weekly benefit of $400 should you be unable to work. OOIDA members 18 or over but not yet age 59 are eligible. (click for more information....) |

OOIDA Group Dental Benefit Plans We offer two Dental Plans - the Value Plan and the Premier Plan. Both plans allow you to use any dentist of your choice, cover 100% of prevailing fees for preventive services, and an 800 number for claim inquiries. These plans have a maximum benefit of $1,000 per covered person, per year. The Value Plan provides preventive and basic dental benefits. The Premier Plan provides preventive, basic, and also includes benefits for major services. (click for more information....) |

OOIDA Group Life Insurance Plan This program offers coverage from $10,000 to $500,000 in increments of $10,000. Your spouse can apply for up to 50% of your approved amount. We also offer a dependent life policy which covers your spouse and children for the low monthly cost of $4.75. This coverage provides $10,000 for your spouse and $5,000 for each child. *There is a special $500,000 Accident Plan, developed for Texas residents in order to meet the requirements of the trucking deregulation law passed in 1995. |

All three policies offer 100% of reasonable and customary charges for medical expenses incurred due to an occupational accident with NO DEDUCTIBLE. Coverage includes accidental death, short-term and long term disability and some limited accidental dental benefits. There are also limited benefits for non-occupational accidents. |
