OOIDA Group Life Insurance Plan

This is a term life insurance plan underwritten by The Prudential Insurance Company of America. Rates are based on age and the amount of coverage requested. Members, including eligible spouses, can apply for $10,000 to $500,000 worth of coverage, in increments of $10,000. Spouses who are not eligible members can apply for up to 100% of the amount the primary member has in force in increments of $5,000 (not to exceed $250,000). We also offer a Dependent Life plan ($10,000 for spouse and $5,000 for children) for $4.75 a month. This option is for a spouse not involved in the trucking industry, and eligible dependent children. Dependent children are covered up to age 19 or to age 25, if a full-time student.

Please Note: Dependent Life Insurance is only available if the member is insured. The combined amounts of the spouse's life insurance cannot exceed 100% (or a maximum of $250,000) of the combined amounts of the member's life insurance unless the primary member's age causes a reduction in coverage.

To be eligible for insurance, a person must meet the conditions of both 1. and 2. below:

1. A person must be a current, dues paid member of the Policyholder. As defined by Owner-Operator Independent Drivers Association membership provisions, a person must be either:

a. Truck Owner Operator: Any individual who is a current, dues paid member of the association who owns or operates one or more commercial truck(s). An individual who owns a commercial truck need not actually drive the truck; or

b. Non-truck Owner: Any individual who is a current, dues paid member of the association who works in a trucking industry-related job such as, but not limited to, truck driver, dispatcher, mechanic, and office staff.

2. A person in 1a or 1b must also:

a. be regularly scheduled to work at least 30 hours per week, or an average of 1500 hours per year;

b. be a citizen or legal resident of the United States, its territories, or Canada;

c. not be a temporary or seasonal employee; and

d. not be a full-time member of the armed forces of any country.

*Limited time offer!!! New members have 60 days from their membership effective date to enroll for up to $50,000 worth of guaranteed coverage. There are no medical questions asked! Spouses of new members would be able to get a maximum of $35,000 in guaranteed coverage.

*at age 70 there is a graded reduction in the coverage amount to 65% and 50% at age 75 and over.

Rates may change as the insured enters a higher age category, also rates may change if plan experience requires a change for all insureds.

Figure your monthly rate:

The rate for a 40-year old member is $2.27 per $10,000. For $50,000 worth of coverage, multiply $2.27 by 5.  The monthly rate would be only $11.35.

*These forms are only for new Members within 60 days of their membership effective date. All other members will need to contact the Medical Benefits Group for an application.

Renewability: As long as the Group Insurance Policy remains in force, and you are actively at work in a class eligible for coverage, you can keep your coverage in force by paying your premiums when due.
