| OOIDA Safe Driving Award Program Guidelines |
The Owner-Operator
Independent Drivers Association Safe Driving Award Program is
designed to recognize and reward OOIDA members for their safe,
accident-free years while operating a commercial vehicle. Safe
driving awards are available to all eligible OOIDA members who
qualify based upon the number of years for which the member has
operated a commercial vehicle without being involved in a preventable
accident.

| Who is eligible? |
All OOIDA members
whose dues are current are eligible to request an award.
|
| How do I qualify for an award? |
You must have no preventable
accidents and must actively drive a commercial vehicle as your primary
occupation during the year(s) for which an award is requested
to qualify to receive an award.

| What is considered a preventable accident? |
For the purposes of
the program a preventable accident is defined as: "any accident
that occurs while operating a commercial vehicle in which the
driver failed to make all reasonable efforts to avoid." In
the event that an award is requested covering any years(s) in
which an accident occurred, a committee comprised of staff experts
and members of the OOIDA Board of Directors will conduct a review
of the circumstances and apply an interpretation of the definition
to determine whether you qualify for an award. To assist the committee
in their determination, you must provide a detailed explanation
of the accident(s) and as much supporting documentation as possible
(police reports, photographs, reconstruction reports, etc.).

| What happens if I have a preventable accident? |
Any year in which
a preventable accident occurs will be excluded from your cumulative
award-years. Once another full award-year is completed in which
no preventable accident(s) occurred, that year will be added to
your cumulative total and you will qualify to receive a new award
designating all cumulative award-years.

| When does my award year begin? |
Your membership anniversary
date and award-year anniversary date are the same. You must provide
the required information covering the entire award-year for any
year(s) for which an award is requested.

| Can I get credit for all previous accident-free years? |
Yes. When first enrolling
in the OOIDA Safe Driving Award Program you may request an award
for all of the previous years driven without a preventable accident.
You need not have been a member during those years, however you
must have been actively driving a commercial vehicle as your primary
occupation during all years for which an award is requested. You
must subtract from the total any year(s) in which a preventable
accident occurred and any year(s) that you were primarily employed
in an occupation other than commercial driving.

| How do I get my award? |
You will not automatically
receive an award. You must fill-out the enrollment form provided,
attach the required verification covering the entire year(s) for
which an award is requested and return them to the OOIDA Membership
Department.

| What verification must be provided? |
Acceptable verification
includes:
1) a letter from the motor carrier(s) under whose authority you were operating stating that you had no preventable accidents during the period; or 2) insurance loss records covering the year(s) for which an award is requested; or 3) any combination of the two. |
| What if I can't provide the verification? |
You must provide the
required verification covering as many years as possible and
a copy of your motor vehicle record from your licensing state.
For the period(s) of time for which verification is unavailable
you must provide a detailed letter of explanation, including the
dates in question and the name and address of the motor carrier(s)
under whose authority you were operating. All letters will be
reviewed by a committee comprised of staff experts and members
of the OOIDA Board of Directors.

| What awards are available? |
Full dues-paying annual
members, multiple-year members and lifetime members will receive
one Safe Driving Award patch and one certificate free of charge
each qualified year. Associate members ($10 spouse or driver members)
must purchase available awards. The award will indicate the number
of verified cumulative safe driving years achieved. Decals, lapel
pins and extra patches are available for purchase. "OOIDA
Gear" (shirts, jackets, hats, etc.) can also be ordered with
your safe driving award stitched on each item.

| Can I trade an award issued under another program? |
Yes. You can trade-in
patches or safety certificates from other programs for the equivalent
OOIDA Safe Driving Award.

| As a fleet owner or motor carrier can I get OOIDA Safe Driving Awards for my drivers? |
Yes, as long as they
are eligible members of OOIDA. As the fleet owner/motor carrier
you are responsible for determining that your employed drivers
and/or leased owner-operators qualify for an award. OOIDA will
verify the membership status of each of your drivers once your
award request is received. You may choose to base your program
on the period of time each driver has been under your direction
(employed by, or leased to you) or you may present awards signifying
all verifiable prior years. If you choose the latter, it is your
responsibility to verify that your drivers were actively driving
a commercial vehicle as their primary occupation and that no preventable
accident(s) occurred during any prior years for which an award
is requested. Please remember that the award-year is determined
by the membership anniversary date of each driver, not the period
of time they are under your direction. In the event that a questionable
accident occurs (or is discovered during the verification process),
you may request that OOIDA review the circumstances and interpret
whether the accident meets the program definition of a preventable
accident.

| How do I get awards for my drivers? |
Please review the
guidelines carefully then complete and sign Section 2 of the enrollment
form attesting that each driver listed qualifies to receive an
award under the guidelines for the number of years specified.
Also complete the order form indicating the year designation,
number and type of awards you wish to present, and return all
forms to the OOIDA Membership Department with your payment.

| Safe Driving Award Recognition items |
An assortment of recognition items are available to honor individual years or milestones in the member's safe driving career. Full dues-paying annual members, multiple-year members and lifetime members who apply and qualify for the OOIDA Safe Driving Award will receive the following free-of-charge for their respective award year:

Additional patches and certificates are available for purchase.
Any "OOIDA Gear" item (shirts, jackets, hats, travel bags, etc.) can also be ordered with your safe driving award stitched on the item. For a small additional fee of $3.00, your OOIDA Gear item can even be personalized with your name. |
