This is a term life insurance plan issued by Puritan Life Insurance Company of America (PLICA). Administrative offices PO Box 11823 Winston-Salem, NC 27116. Rates are based on age and the amount of coverage requested. Members, including eligible spouses or domestic partners, can apply for $10,000 to $250,000 worth of coverage, in increments of $10,000. Spouses or domestic partners who are not eligible members can apply for up to 100% of the amount the primary member has in force in increments of $5,000 (not to exceed $125,000). We also offer a Basic Dependents Term Life coverage ($10,000 for spouse or domestic partner and $5,000 for children) for $11.40 a month. This option is for a spouse not involved in the trucking industry, and eligible dependent children. Dependent children are covered from 14 days up to age 21 (or 25 if a full-time student).
Please Note: Dependent Life coverages are only available if the member is insured. The combined amounts of the spouse’s or domestic partner’s life insurance cannot exceed 100% (or a maximum of $125,000) of the combined amounts of the member’s life insurance.
To be eligible to enroll or apply for insurance, an individual must be between the ages of 18 and 64 and be a current, dues paid member of OOIDA and:
- Own or operate one or more commercial truck(s), or
- Work in a trucking industry related job such as, but not limited to, truck driver, dispatcher, mechanic or office staff.
- If a spouse is applying and meets the member eligibility requirements, he/she may apply for the same amounts as a member. He/she will be required to pay $10 annual OOIDA membership dues.

*at age 70 there is a graded reduction in the coverage amount to 65% and 50% at age 75 and over.
Rates will change as the insured enters a higher age category. Also, rates may change if plan experience requires a change for all insureds.
Figure your monthly rate:
The rate for a 40-year old member is $2.72 per $10,000. For $50,000 worth of coverage, multiply $2.72 by 5. The monthly rate would be only $13.60.
Effective date: Coverage will be effective the first day of the following month after you enroll or after underwriting approval, if necessary, subject to the actively at work requirements or dependents deferment.
Renewability: As long as the Group Insurance Policy remains in force, and you are in a class eligible for coverage, you can keep your coverage in force by paying your premiums when due.
